NOTE (for Students under 18): Public high schools now require our academy to request their withdrawal form. Use our Intent to Enroll/Withdrawal Request Form to request the withdrawal form for the previous high school. We must have the previous school's withdrawal form on our records in case law enforcement or other state officials ask for proof. 

The previous school's withdrawal form proves that you have officially informed them that your child will attend our academy. Important: make sure you put the name of our academy and our info (as shown below) on their withdrawal form. This tells them exactly where the student plans to enroll and attend. If you don't withdraw from the previous school, your child will still technically be a student of that school. The student must be withdrawn from the previous school first BEFORE we can register/enroll them.
